RULES AND REGULATIONS CONCERNING BOTOX / JUVÉDERM CLUB
- Only NEW Blair Plastic Surgery Botox or Juvederm patients are eligible for $50 credit towards their first Botox or Juvederm treatment. The $50 discounted treatment does NOT count toward the regular Botox Club Specials; i.e.,it does NOT count as the first treatment towards buying 3 at regular price and getting fourth treatment free.
- New Blair Plastic Surgery Botox or Juvederm patients MUST show proof that they have had this treatment done elsewhere in order to be eligible for $50 credit. If they cannot provide proof or have never had this treatment done before, they are eligible to join the "Botox Club", but cannot receive the $50 credit toward their first treatment.
- Existing Blair Plastic Surgery Botox and/or Juvederm patients are instant members of the new Botox Club as of October 1, 2010. Treatments after this date automatically count toward "Buy 3 get one Free" but are not eligible for the $50 discount.
- No mixing of Botox and Juvederm treatments to obtain free treatment. Purchasing 3 Botox sites makes you eligible for a fourth free Botox treatment ONLY and/or purchasing 3 Juvederm syringes makes you eligible for a fourth free syringe ONLY.
- This offer is valid for one full year from the date of the first Botox or Juvederm treatment and Free treatment MUST be used within that year.
- Botox Club Card will be distributed to patients to help patients keep track of their treatments. We also keep track for you through our office practice computer software.
