Male breast reduction surgery is the fourth most popular cosmetic surgical procedure among men. Nearly 17,000 male breast reduction procedures were performed in 2009, a 50 percent increase since 1997. The majority of these procedures were performed on men ages 19-34.
Gynecomastia, or enlarged male breasts, occurs as a result of a hormonal imbalance in which testosterone levels are decreased relative estrogen levels. Several common causes of this imbalance include: natural hormone changes, certain health conditions, exposure to anabolic steroids or estrogen, side effects of some medications or street drugs, and certain cancer treatments. However, in approximately 25 percent of boys and men the exact cause remains unknown.
Depending on the cause, gynecomastia often resolves on its own or will resolve after treating any underlying medical conditions. In other times, surgical treatment may be necessary. Generally, liposuction is used to remove excess fat from the breasts, breast tissue can be directly excised or a combination of the two. However, liposuction of the male breast can be difficult due to the vascular and fibrous nature of the breast tissue. And when excisional surgery is necessary, it leaves permanent scars that some men find objectionable.
Dr. Robert Louton often uses ultrasound assisted liposuction (UAL) to liquefy the fat before it is suctioned out, which results in less trauma to the surrounding structures such as blood vessels and connective tissues. This technique is particularly beneficial in treating areas of fibrotic tissue interspersed with fatty deposits, such as breast tissue in males. There is less bleeding, less bruising and improved ability to contour the breast. Using UAL may, in some instances, eliminate the need for excision of tissue and subsequently eliminate the scars associated with it.
